PDF Medication Request Forms
- Make sure that you have Adobe Acrobat Reader installed on your
computer. If not, you can download it for free at Adobe.
- Click on the link leading to the PDF document you wish to save.
- After the document has opened in Adobe Acrobat Reader, click on
the "Save a Copy" icon found at the top-left of the screen. A dialogue
box will appear.
- Using the drop-down menu next to "Save In" of the dialogue box,
navigate to the folder in which you wish to save your document.
- Once you have chosen the location, click the "Save" button. The
document is now in its new location and can be accessed anytime. This
"Save" function only saves the blank form, completed forms cannot
be saved.
