The Path to Health Pilot Project provides access to primary care for up to 25,000 undocumented adults ages 21-64 who are enrolled in an emergency services only (restricted scope) Medi-Cal program aid code and reside in one of the 35 CMSP counties. The project launched February 2019 and there are currently 46 clinics providing care under the Path to Health program. Additional Clinics interested in participating should see below and submit an application by 9/5/2019 at noon.
Request for Application – Provider Participation – Phase 2
The Lead Agency Applicant for the Path to Health Pilot Project – Phase 2 must be a Federally Qualified Health Center (FQHC), Tribal Health Clinic, or Rural Health Center (RHC) that has an existing provider contract with the Governing Board to serve CMSP members or will, at the time the Pilot Project commences, have a provider contract with the Governing Board. Lead Applicants must demonstrate evidence of Partner Collaboration and Support with both the County Health Department and the County Social Services Department in the county or counties to be served through the Pilot Project.
